
Road traffic crash is not only a global pandemic that kills more than a million people a year but has been a major concern in majority of countries, including Malaysia. In a study conducted by the MIROS, an average of 400,000 crashes are reported annually. Many argue that the term incident is more appropriate as when an “accident” occurs, as “accidents” implies that the even was related to fate or chance. However, when the root cause is determined, it is usually found that many events were predictable and could have been prevented if the right actions were taken. Therefore, there is an importance to investigate these incidents as it, most importantly, finds out the root cause of the incidents and to develop countermeasures regarding minimizing human and economic impact from road crashes. To ensure that the investigation is done competently, the aspects of the investigations are multidisciplinary, comprising of in-depth inspection on road design and environmental factors, vehicle exterior and interior damage, and on occupant safety and injury mechanism. Competent and trained personnel with knowledge on performing a competent investigation will ensure proper life-saving policies and systems are in place to safeguard the organisation’s properties but more importantly, their employee’s lives.
